Effective January 1, 2013, prescription drug benefits for certain eligible plan participants
Who are age 65 and over and covered by Medicare will be provided by SilverScript® Insurance Company,
a subsidiary of CVS Caremark. SilverScript specializes in managing prescription drug benefits for the
Medicare-age population and is an approved Medicare Part D Prescription Drug Plan provider.

NEW prescription drug informational mailings, including an important
package mailed to your home at the end of October.
This program is comprised of two parts:
An employer group prescription drug program which is approved the Centers
for Medicare and Medicaid Services (CMS), called a group Medicare part D
Additional wrap benefits, which provide enhancements to the group Medicare
part D program to mirror the 2012 prescription drug benefits.
By combining these two programs, the 2013 prescription drug program provides
the same level of benefits available in 2012, which minimizes disruption to you.
What Do I Need to Know
Don’t “Opt Out”! Your retiree coverage includes both medical and
prescription drugs. Don’t “opt out” of SilverScript if you want to keep medical
coverage for 2013. If you “opt out” of the SilverScript® prescription
plan, you will lose both medical AND prescription drug coverage for
2013. If you opt out, you will not be able to reenroll in medical and
prescription drug coverage until the 2013 Annual Enrollment period,
with coverage effective for 2014. NOTE: If you retired prior to January
1, 2010 from UST or one of its affiliates or subsidiaries, you cannot
reenroll in your Plan if you opt out of coverage for 2013.
Don’t Worry! If you are currently covered in the Caremark program, you will
be automatically enrolled in the SilverScript® Medicare prescription drug
program effective January 1, 2013. The Altria Group Benefits Center will
automatically transfer your prescription drug coverage to SilverScript – so
long as the Benefits Center has your Health Insurance Claim Number (HICN),
also called your Medicare claim number, and a street address on file. This
information is required by CMS. You will receive a separate notice from the
Benefits Center if either your HICN or street address is not on file. It is very
important for you to provide your HICN and/or street address to the Benefits
Center, if they request it from you, by calling 1-800-872-3777 . Do not contact
SilverScript to enroll – the Benefits Center will transfer your enrollment to
(If you are eligible but not currently enrolled in retiree medical coverage and
decide to enroll for 2013, you must contact the Benefits Center during Annual
Enrollment (October 2-18) at 1-800-872-3777 ).

Options for Filling Your Prescription
You may continue to have your prescriptions filled at CVS Caremark network
retail pharmacies or CVS Caremark mail order service.
CVS Caremark Network Retail Pharmacies - Just as in 2013, for 2014 you
will continue to have access to more than 68,000 CVS Caremark network
pharmacies nationwide. You may fill your prescriptions at a non-network
pharmacy; however, you may have to pay more. To find a pharmacy go to
Pharmacy Locator on this site up and to your left.
CVS Caremark Mail Order Service – You may choose to have your long-
term or maintenance prescriptions filled and mailed directly to your home
using the CVS Caremark mail order service. Mail order service is both
convenient and cost effective. With mail order service, you receive up to a
90-day supply of your medication for less than the price you would pay for
three 30-day supplies at a network retail pharmacy. To order maintenance
prescriptions through mail order, go to www.caremark.com or call 1-877-878-5717 .
Note: Participants enrolled in the coinsurance plan may continue to have a 90-day
supply of long-term or maintenance drugs filled at CVS, with the same
benefits as through mail order. Participants in the copayment plan can only fill a
90-day supply of long-term or maintenance drugs at mail order, not at CVS.

Many plans cover generic drugs at 100% through the Caremark mail order
service for 90-day supplies of covered long-term or maintenance prescription
Caremark mail order service checks the availability of U.S. Food and Drug
Administration-approved, generic substitutions to fill prescriptions when
authorized by physicians and permissible by law. By using generics, you may
reduce your prescription drug cost. Talk with your doctor or pharmacist about

To obtain a list of covered drugs, also called a formulary, go to formualry below for Med D and current commercial PDL for wrap.
Remember, certain drugs which are not on the Medicare formulary are not covered in a standard Medicare prescription
drug plan. However, because Altria Client Services’ prescription drug plan is enhanced
beyond the standard Medicare prescription drug benefit, you will be able to fill the same drugs in 2013 and
pay the same copayment or coinsurance
as in 2012. Of course, the formulary may change periodically, so be sure to
check the most recent list of covered drugs. Call SilverScript Customer Care if
you have any questions.
Note: Even though the Altria Client Services retiree medical plan covers some
drugs which are not covered by Medicare, SilverScript is required by Medicare to
send you a letter if your drug is not on the standard Medicare formulary.

Large Pharmacy Network – More than 68,000 retail pharmacies
nationwide make up the CVS Caremark pharmacy network. This is the same
retail pharmacy network of offered to you in 2012, so it is familiar to you.

For example, the Plans may be amended to change the percentage of employer,

including the level or type of prescription drug coverage. Any Participating
Company may change or terminate, in whole or in part, the participation of its
